Method and severity

Laparoscopic appendectomy is modeled with a KRW 1,800,000 covered surgery fee and 3 default inpatient days. Open appendectomy uses KRW 1,300,000 and 4 days. Single-port laparoscopy uses KRW 2,100,000 and 3 days because the source model treats its materials and approach as a higher covered fee.

Severity changes both fee and admission. Simple appendicitis uses multiplier 1.0 and 3 days, suppurative or gangrenous appendicitis uses 1.15 and 4 days, and perforated appendicitis with peritonitis uses 1.4 and 6 days.

Emergency and non-covered items

If the patient comes through the emergency room, the covered emergency-management fee is KRW 50,000 at clinics, KRW 100,000 at general hospitals, and KRW 150,000 at tertiary hospitals. In the source model, night or holiday emergency surgery applies a 30 percent surcharge to the surgery and anesthesia bundle.

The calculator also preserves non-covered add-ons: PCA pain control at KRW 100,000 to KRW 200,000 and adhesion barrier at KRW 150,000 to KRW 400,000. Ward full fees are KRW 130,000 per day at clinics, KRW 160,000 at general hospitals, and KRW 190,000 at tertiary hospitals.

Copay and insurance

Adult inpatient surgery uses a 20 percent NHI copay. Children aged 15 or younger use a 5 percent inpatient copay. Low-income status uses 14 percent and medical aid uses 10 percent when those overrides apply.

Private indemnity generation is then applied to covered copay, non-covered PCA or adhesion barrier, and upper-room surcharge. The tax-credit step keeps the Korean 3 percent of salary threshold and 15 percent credit rate.
