Treatment fee table

Medication uses KRW 50,000 as a monthly covered outpatient base. TURP uses a KRW 3,000,000 covered full fee and 3 default inpatient days. HoLEP uses KRW 4,800,000 and 3 days. PVP or green-light laser uses KRW 3,500,000 and 2 days.

UroLift is modeled as non-covered, with KRW 3,500,000 to KRW 8,000,000 and KRW 4,800,000 average for the 4-anchor baseline. Rezum water-vapor therapy is also non-covered, with KRW 2,500,000 to KRW 6,000,000 and KRW 3,500,000 average.

Size and add-on logic

The Korean model changes non-covered minimally invasive cost by prostate volume. Under 30 cc uses multiplier 0.85, 30 to 60 cc uses 1.0, 60 to 100 cc uses 1.25, and over 100 cc uses 1.5. UroLift anchor count also changes cost: 2 anchors use 0.75, 4 anchors use 1.0, and 6 anchors use 1.25.

Combined bladder-stone removal adds KRW 600,000 as a covered add-on, and combined urethral-stricture incision adds KRW 400,000. PCA uses KRW 100,000 to KRW 200,000, and upper-room daily surcharge is KRW 100,000 at clinics, KRW 170,000 at general hospitals, and KRW 250,000 at tertiary hospitals.

Insurance interpretation

Inpatient covered surgery uses a 20 percent copay. Outpatient covered care uses 30 percent at clinics, 50 percent at general hospitals, and 60 percent at tertiary hospitals. UroLift and Rezum can preserve ejaculation but are non-covered in the source model, so claim risk must be checked against the policy.
