This is a Korean special cash benefit, not family caregiver wages
Korea's long-term care family care cash benefit is a special cash benefit paid by the National Health Insurance Service when an insured beneficiary cannot reasonably use ordinary home or facility benefits for a narrow statutory reason.
It is not family caregiver wages paid under an employment contract with a long-term care provider, and the family member does not have to become a certified care worker merely to apply for this cash benefit.
The calculator separates those two systems so that a provider's family-care payroll offer is never mistaken for the statutory monthly cash amount.
What the calculator estimates
It turns recognition status, the selected statutory reason, an NHIS decision, valid dates, suspended periods, other benefits, and document readiness into an eligibility signal and a month-by-month 2026 estimate.
What remains an NHIS decision
NHIS determines whether the address or personal condition satisfies the legal reason, whether evidence is sufficient, and the official first and last payable dates.
The five statutory reason groups
Article 24 of the Long-Term Care Insurance Act and Article 12 of its Enforcement Decree define exceptional situations in which family care cash benefit may be considered.
A household preference for family care, a waiting list, or dissatisfaction with a nearby provider does not by itself establish one of these legal reasons.
1. A designated island or remote area
The beneficiary lives in an island or remote area designated by the Ministry of Health and Welfare because long-term care providers are materially insufficient.
Match the exact registered address to the current annex of Ministry Notice 2025-173 and confirm it with the NHIS branch.
2. A recognized disaster or similar event
A natural disaster or comparable situation recognized by the Ministry makes ordinary benefits difficult to use.
Keep notices and records that identify both the event and the period during which care access was disrupted.
3. Infectious disease transmission risk
The beneficiary is an infectious-disease patient who presents a risk of transmission to another person under Korean infectious-disease law.
Medical evidence and review by the long-term care grade decision committee may be required before NHIS confirms the reason.
4. Registered mental disability
The beneficiary is currently registered as a person with a mental disability under the Welfare of Persons with Disabilities Act.
Prepare proof of current registration as well as the long-term care recognition details.
5. Avoidance of interpersonal contact due to physical deformation or a similar reason
The beneficiary avoids interpersonal contact because of physical deformation or another qualifying physical condition.
A personal statement alone does not automatically confirm the condition, so ask NHIS which medical evidence and committee review are required.
2026 amount and partial-month calculation
From January 1, 2026, Ministry of Health and Welfare Notice 2025-247 Article 79 sets the family care cash benefit at KRW 240,450 per month.
Twelve full payable months total KRW 2,885,400.
The monthly amount does not increase or decrease by long-term care grade, although valid long-term care recognition remains a basic condition.

Each partial month uses its own number of calendar days
Enforcement Rule Article 20 requires day proration when the payment reason begins or ends during a month.
This planning tool rounds each monthly result to the nearest won, while the official NHIS payment statement controls any final rounding difference.
It calculates only the part of the selected period that overlaps 2026 and does not assume a rate for a later year.
A practical application sequence
- Check the long-term care recognition notice for current status, grade, and validity dates.
- Identify which one of the five statutory reason groups matches the beneficiary's facts.
- Prepare the family care cash benefit application and reason-specific evidence for the relevant NHIS branch.
- For infection risk or a physical-contact reason, ask whether a medical certificate and committee review are required.
- After NHIS issues its decision, enter the official payable dates rather than the date on which the household first began informal care.
- If the reason ends or another benefit starts, check the change-report deadline immediately.

Interaction with other long-term care benefits
Enforcement Rule Article 17 generally prevents family care cash benefit from being paid at the same time as ordinary home benefits, facility benefits, or another special cash benefit.
An exception exists for permitted other home benefits, including eligible welfare equipment, so the calculator does not treat that choice like ordinary visit care or facility admission.
It also does not subtract welfare-equipment cost from KRW 240,450, because the equipment's eligibility and annual cap follow separate rules.
Check the 14 days change-report window
Enforcement Rule Article 20 requires a change application when the payment reason ends or relevant circumstances change.
The calculator shows a reference date 14 days after the entered reason-end date.
Contact NHIS as soon as the facts change because the official triggering event may differ from the household's assumed date.

Confirming a remote-area address
Match the complete legal address and move-in date to the notice instead of relying on the town name alone.
Enter only dates that overlap both actual residence and valid long-term care recognition.
Planning after discharge
Ask whether medical evidence clearly states the transmission-risk period and how long committee review may take.
Use the expected end date to prepare for a timely change report.
Approaching recognition renewal
If the requested period extends beyond recognition validity, the calculator limits the estimate to the overlap and shows a warning.
Recalculate with the renewed dates to identify any gap.
Changing to visit care or a facility
Test the day before the new benefit begins as a possible cash-benefit end date, then confirm the transition with NHIS.
Do not assume that two benefits can cover the same day.
