Plan Vyndamax costs using Korean coverage rules
ATTR-CM is transthyretin amyloid cardiomyopathy, a disease involving amyloid deposits in the heart.
Vyndamax contains tafamidis, and this calculator organizes household costs for a specialist-prescribed 61 mg capsule once daily.
It does not decide whether to start, combine, change or stop treatment, or how long treatment should continue.
This page uses South Korean National Health Insurance rules for care within calendar year 2026.
The MFDS product page checked November 2, 2026 lists product code 648903510 and a public reference price of KRW 100,000, effective March 1, 2025.
A public reference is not a provider’s actual non-covered quote or a confidential risk-sharing agreement price.
Three different financial questions
- Payment before relief: the annual medicine and care amount you may have to fund first.
- Annual net medical cost: the amount after incremental ceiling relief and confirmed support.
- Additional national tax credit: a separate employee tax estimate, not cash available to pay today’s bill.
Compare three drug-payment paths
Covered with special registration
Assumes both drug coverage and applicable special-case registration; the covered drug share is 10%.
Covered with ordinary copay
Assumes drug coverage without the special rate; replace the 30% pharmacy reference with the actual confirmed percentage.
Full drug cost
Assumes the patient pays the entire drug quote, such as outside reimbursement criteria; that drug payment is excluded from the ceiling.
Drug reimbursement and special-case registration are separate questions.
Missing registration alone does not automatically mean a 100% drug charge, and a hospital not yet stocking the product does not itself establish a non-covered billing classification.
Ask separately about supply, the prescription route, benefit criteria and your patient share.
Unknown evidence leaves all results conditional; a star only identifies the path matching the statuses you entered, not an approval by this website.
Records to discuss with your specialist
HIRA Notice No. 2026-69, effective April 1, 2026, distinguishes hereditary and wild-type pathways for confirmed ATTR-CM in adults aged 18 or older.
These are discussion checkpoints rather than an invitation to interpret imaging or diagnose yourself.
Initial reimbursement evidence
- The hereditary pathway requires genetic-test evidence of the hereditary diagnosis.
- Wild-type criteria include all of: sustained NYHA I–III during the previous six months, diagnosed heart failure and at least one heart-failure admission within six months, NT-proBNP ≥ 600 pg/mL, and end-diastolic left-ventricular wall thickness ≥ 12 mm.
- Diagnostic evidence includes suspicion on ECG, echocardiography or CMR, AL amyloidosis excluded by blood and urine testing, and grade 2 or 3 cardiac uptake on the specified nuclear imaging or cardiac-tissue confirmation of TTR amyloid.
- Liver or heart transplantation and a mechanical cardiac assist device are exclusion criteria.
Continuation and concurrent therapy
Response assessment is required before treatment and every 12 months afterward.
Stopping criteria include the specified exclusions, progression to NYHA IV, an absolute wall-thickness increase of at least 2 mm, or an absolute LVEF decrease of at least 5 percentage points compared with the preceding assessment.
Notice 2026-69 does not reimburse concurrent use with vutrisiran and requires a heart-related specialist prescription and objective documentation.
Your treating team interprets these criteria; never interrupt treatment because of a calculator result.
Enter days, capsule prices and actual care copays
Days within 2026
Enter 0–365 prescribed days falling between January 1 and December 31, 2026; the model assumes one prescribed 61 mg capsule per day and does not recommend extending treatment.
Two capsule prices
Keep the covered unit price separate from the full-cost provider quote; the default full-cost price merely repeats the public reference and must be replaced when an actual quote is available.
Additional covered care
Enter the annual patient copay for related covered visits and tests, not the gross medical bill; the same care amount is held constant in all three comparisons and must not duplicate the drug cost.
Non-covered medical expenses
Enter eligible medical spending from quotes or receipts; transport, ordinary caregiver charges and lost earnings do not belong in the medical tax-credit input.
A twelve-month course starting in November crosses two ceiling years and must not use one annual ceiling for the entire course.
Recalculate the next calendar year using its own prices and rules.
Zero default care or support amounts mean no extra amounts were entered, not that treatment is universally free of examination charges.
Only the additional ceiling relief belongs to this plan
The ceiling under National Health Insurance Act Article 44 and Enforcement Decree Article 19 applies per patient and calendar year.
Do not combine relatives’ copays or include non-covered charges, full drug payments, selective benefits or other excluded charges in eligible spending.
Choose the income decile determined by NHIS premiums rather than inferring it from salary alone.
Incremental formula
Additional relief = max(B + N − C, 0) − max(B − C, 0)
B is this patient’s other eligible copay in 2026, N is this plan’s eligible drug share plus the entered covered care copay, and C is the annual ceiling.
Full drug charges are excluded from N, although separately entered covered care may still count.
The second term prevents an existing refund from being attributed to this new drug budget.
2026 annual NHIS patient ceilings in KRW| Premium decile | General ceiling | Nursing hospital over 120 days |
|---|
| 1 | 900,000 | 1,430,000 |
| 2–3 | 1,120,000 | 1,810,000 |
| 4–5 | 1,730,000 | 2,450,000 |
| 6–7 | 3,260,000 | 4,040,000 |
| 8 | 4,460,000 | 5,800,000 |
| 9 | 5,360,000 | 6,980,000 |
| 10 | 8,430,000 | 10,960,000 |
The 120-day distinction concerns nursing-hospital admissions, not capsule days.
Relief can involve provider settlement and later refunds; amounts already settled are not paid again, so the result is not a promise of a new cash refund.
Final premium classification, excluded bills and deductions for insurance arrears can change the settlement.
Worked 365-day example
No extra care, other copays or support; general decile 4–5
- KRW 100,000 × 365 = KRW 36,500,000 total drug cost.
- The 10% drug share is KRW 3,650,000; 30% is KRW 10,950,000; full payment is KRW 36,500,000.
- Against the KRW 1,730,000 ceiling, additional relief is KRW 1,920,000, KRW 9,220,000 and zero for the drug respectively.
- Annual net cost is KRW 1,730,000 in either covered path versus KRW 36,500,000 in the full-drug path, a KRW 34,770,000 difference.
- Repeating those annual budgets five times gives KRW 8,650,000 versus KRW 182,500,000.
At decile 10, the general ceiling is KRW 8,430,000, so the 10% drug share generates no relief, while the ordinary path generates KRW 2,520,000 relief.
Equal net costs at a lower ceiling do not mean equal upfront payments or cash needed while waiting for settlement.
Turn off ceiling relief to examine a budget before assuming it applies.
Confirmed support and employee tax-credit increases
Vyndamax appears in a published full-payment patient support contact list for risk-sharing drugs, but this does not establish a universal refund rate or your eligibility.
Confirm the patient, prescription, period and payable amount, and enter a distinct amount for each path.
Support must be a confirmed payment against cost remaining after NHIS settlement, without overlapping ceiling relief.
National tax estimate under Income Tax Act Article 59-4(2)
The model applies 15% to qualifying medical expenses above 3% of the claiming employee’s gross salary.
The ordinary eligible-base cap is KRW 7,000,000; protected categories include the employee themself, a person aged 65 or older at year-end, disability and applicable verified statutory special cases.
Confirm the exception for this patient before selecting it.
Other ordinary net expenses use the 3% threshold first, and any unused threshold reduces protected expenses.
The result is total credit with this plan minus the existing credit, after removing ceiling relief, support and indemnity reimbursement.
Already reimbursed expenses must not be claimed again.
For salary KRW 50,000,000, no other expenses and net medical cost KRW 1,730,000, the threshold is KRW 1,500,000 and the illustrative additional national credit is KRW 230,000 × 15% = KRW 34,500.
This is not a cash benefit for a patient without employee tax liability.
Actual payer eligibility, tax liability, other credits, the standard-credit choice and later refund corrections require review; local income tax is not included.
Use the calculator before the billing discussion
- Gather the prescription and provider quote; confirm Vyndamax 61 mg, days within this calendar year and the unit price.
- Ask about drug coverage and special registration separately, keeping unresolved evidence unknown.
- Enter additional actual covered copays, non-covered medical costs and the same patient’s other eligible annual spending without duplicating drug charges.
- Select the NHIS decile and nursing-hospital category, then enter non-overlapping support confirmed for each path.
- If needed, enter the claiming employee’s salary, other net medical expenses and the patient’s verified cap exception.
- Review upfront and net costs together, save the assumptions as a TXT budget and discuss them with the provider and NHIS.
Cost inputs are processed in your browser, and the budget needs no patient name or medical-record number.
Recalculate when the quote or registration period changes rather than reusing an old result.
Product boundaries and five-year interpretation
This model covers only prescribed Vyndamax 61 mg.
Vyndaqel 20 mg requires separate confirmation of formulation, labeled ingredient, indication and reimbursement; do not substitute products or convert capsule counts yourself.
Other ATTR treatments, transplantation costs and health effects of treatment delays are outside the calculation.
Useful planning situations
- Before starting, compare the full-payment cash requirement with the net cost under confirmed coverage.
- After substantial other medical spending, enter prior eligible copays to isolate relief attributable to this plan.
- For a family savings discussion, repeat the same annual assumptions five times; this forecasts neither survival nor prescription duration, future prices, registration renewals or future policy.
Frequently asked questions
Does an ATTR-CM diagnosis automatically qualify?
No; the provider must confirm diagnostic evidence, adult age, the hereditary or wild-type pathway, exclusions, specialist prescription and required documentation.
Does missing registration mean full drug payment?
Not necessarily; covered treatment may use an ordinary copay, while outside-criteria drug payment is a separate question.
Does this tool register V900?
No; the designated institution and NHIS determine your actual registration code and validity, and the rate does not extend to all heart disease.
Can non-covered drugs be reduced to the annual ceiling?
No; full drug charges and non-covered medical bills are excluded from the eligible ceiling amount in this model.
Why can the covered paths have equal net costs?
Both may exceed the same annual ceiling; the initial amounts payable and cash needed before settlement can still differ substantially.
Can the calculator determine an RSA refund rate?
No; enter only patient-specific confirmed payments, excluding any amount already counted as ceiling relief.
Will I receive all the displayed tax credit?
The additional national credit is illustrative; actual liability, other credits and payer eligibility affect what can be used, so it is not deducted from the payment budget.
Does five years mean I should take it for five years?
No; it repeats current annual inputs and 2026 assumptions for arithmetic comparison, and treatment and policy must be reviewed independently.
