Generation-specific premium and self-pay

The model compares current generation 1, 2, 3, or 4 against target generation 4 or generation 5. It calculates annual premium if kept, annual premium if switched, covered-medical self-pay, non-covered self-pay, annual total cost, and cumulative cost.

Generation 5 separates severe non-covered treatment and non-severe non-covered treatment. Non-severe non-covered treatment can have a 50% self-pay and an annual KRW 10 million limit, while severe non-covered care uses a different copay rule.
